Originally it was reported a cell phone picture taken by medical personnel or ambulance drivers was thought to be the item that was being used to extort money from John Travolta and his wife Kelly.

But the document in question is actually a "refusal to transport" document which Travolta had to sign in case he decided not to have the ambulance drive Jett to the hospital. At the time, Travolta, a licensed pilot was considering flying Jett back to Florida for medical treatment.
